Why this applies to Tamoxifen Citrate

Tamoxifen Citrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the wrinkle with tamoxifen: it's actually a prodrug. Your body has to convert it into its active form (called endoxifen) using a liver enzyme named CYP2D6. That active form is what does the real cancer-fighting work. So anything that slows down CYP2D6 could leave you with less of the active drug, which may make your tamoxifen less effective over time.

In a lab dish, False Unicorn appears to block CYP2D6, so in theory it could lower your tamoxifen benefit. I want to be honest though: this is only test-tube evidence, not proof in real people, so we don't know how much it matters in practice. Since tamoxifen's job is too important to risk, please check with your oncologist or pharmacist before adding False Unicorn.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the wrinkle with tamoxifen: your body has to convert it into its active form (called endoxifen) before it can fight breast cancer, and the liver enzymes that do this conversion include CYP3A4. In lab dish studies, False Unicorn appears to slow CYP3A4 down. If that held true in people, it could theoretically nudge tamoxifen's activation in a less predictable direction, and because tamoxifen is such an important medicine, I don't like leaving that to chance.

I'll be honest with you though, this is only test-tube evidence, not something proven in real patients, so it's more of a caution than a red alert. Given how much rides on tamoxifen working properly, please loop in your oncologist or pharmacist before adding False Unicorn rather than starting it on your own.

What Is Tamoxifen Used For?

Brand names: Nolvadex, Soltamox

Tamoxifen is used to treat estrogen receptor-positive metastatic breast cancer in both women and men, and as follow-up (adjuvant) therapy after surgery for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.

In women with ductal carcinoma in situ (DCIS — an early, non-invasive breast change) following surgery and radiation, tamoxifen lowers the risk of developing invasive breast cancer. It is also approved to reduce the risk of breast cancer in women who are at high risk but have not yet been diagnosed. Both Tamoxifen Citrate tablets and Soltamox oral solution share these indications.
