Tenapanor Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the good news first: this is a very weak, theoretical concern. The only evidence we have is a lab (test tube) study suggesting False Unicorn might slow down an enzyme called CYP3A4. The thinking goes that if that enzyme is blocked, drugs it processes could build up a bit. That doesn't necessarily play out in real people.

For tenapanor (Ibsrela), this actually matters even less. Tenapanor works right in your gut and is barely absorbed into your bloodstream, so it isn't really cleared by CYP3A4 the way many other drugs are. In practice, I wouldn't expect False Unicorn to noticeably change how it works. The bigger effect to watch is tenapanor's main job, loosening stools, so flag any worsening diarrhea or cramping with your pharmacist or doctor before combining these.

Tenapanor is approved for two distinct conditions, each under a separate brand. Ibsrela is used to treat irritable bowel syndrome with constipation (IBS-C) in adults. XPHOZAH is used to reduce high phosphorus levels (hyperphosphatemia) in adults with chronic kidney disease (CKD) who are on dialysis — specifically as an add-on when phosphate binders haven't worked well enough or aren't tolerated.
