These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Fennel interacts with whole categories of medication. Aspirin, Butalbital, Caffeine falls into 2 of those categories, so it can interact with Fennel in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
1 of 2Why this applies to Aspirin, Butalbital, Caffeine
Aspirin, Butalbital, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fennel interacts with.
In vitro research suggests that fennel inhibits CYP3A4 enzyme activity. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Fiorinal (aspirin, butalbital, caffeine) and fennel. One ingredient, the butalbital, is partly broken down by a liver enzyme called CYP3A4. In test-tube studies, fennel slowed that enzyme down. If the same thing happened in your body, butalbital could clear a little more slowly and linger longer, which might mean more drowsiness, dizziness, or grogginess than usual.
Now the reassuring part: this has only been seen in lab dishes, never in actual people, so it's theoretical. Sprinkling fennel on food or sipping fennel tea is very unlikely to matter. Concentrated fennel capsules or the essential oil are where I'd be more cautious. If you use those regularly, just run it by your pharmacist or doctor first.

ANTICOAGULANT/ANTIPLATELET DRUGS
2 of 2Aspirin, Butalbital, Caffeine also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.
Animal research suggests that fennel oil has antithrombotic and antiplatelet effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The piece of Fiorinal that matters here is the aspirin. Aspirin already thins your blood by making platelets less sticky, which is why people on it can bruise more easily or bleed a little longer from a cut. Fennel may add to that effect. In lab and animal studies, compounds in fennel showed mild antiplatelet, anti-clotting activity, so stacking the two could, in theory, nudge your bleeding risk up a bit.
I want to be honest about how soft this evidence is. It's based on test-tube and animal work, not real patients, and fennel as a food or spice is very unlikely to cause trouble. The bigger question is concentrated fennel supplements or fennel oil. If you're considering those, check with your pharmacist or doctor first, and watch for unusual bruising or bleeding.

“The aspirin inside Fiorinal (aspirin, butalbital, caffeine) is the reason this list deserves careful reading rather than a quick skim. - Digestive upset and bloating
- Infant colic
- Menstrual cramps
- Cough and respiratory complaints
- Breast milk support (traditional)
Fennel is a Mediterranean herb widely used as a food and spice, and traditionally taken for digestive complaints, colic, and menstrual cramps. Some small studies suggest possible benefit for certain stomach and menstrual symptoms, but the evidence is limited. It is generally well tolerated in food amounts, but concentrated supplements and the essential oil should be used with caution and after checking with your pharmacist or doctor.
