Why this applies to Estrogen

Estrogen falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fennel interacts with.

Theoretically, fennel might increase levels of drugs metabolized by CYP3A4.

In vitro research suggests that fennel inhibits CYP3A4 enzyme activity. This effect has not been reported in humans.

Here's the gist: your body clears estrogen using a liver enzyme called CYP3A4. Lab tests (in test tubes, not in people) hint that fennel may slow that enzyme down. If that happened in your body, estrogen could clear a little more slowly and build up, which in theory might mean stronger estrogen effects or more of the usual side effects like breast tenderness, bloating, nausea, or headaches.

The honest truth is this is only based on laboratory work and has never been shown in real patients. Cooking with fennel or sipping fennel tea almost certainly won't matter. Concentrated fennel capsules or fennel essential oil are where I'd be a bit more cautious. If you use those regularly, just run it by your pharmacist or doctor first.

Here's the gist with fennel and your estrogen therapy. Fennel contains plant compounds, like anethole, that can act a little bit like estrogen in the body. The concern on file is theoretical: in large, concentrated amounts, those compounds might compete with your prescribed estrogen at the same receptors. That could, in theory, nudge your hormone levels slightly off and affect how well your therapy controls things like hot flashes or vaginal dryness.

The important part: this is based on lab pharmacology, not real patient reports, so I wouldn't lose sleep over it. Fennel as a food or spice in cooking is very unlikely to matter. The caution is mainly for concentrated fennel supplements or the essential oil. If you're considering those, please run it by your pharmacist or doctor first.
