German Chamomile and Estrogen

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

German Chamomile interacts with whole categories of medication. Estrogen falls into 3 of those categories, so it can interact with German Chamomile in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 3

Why this applies to Estrogen

Estrogen falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group German Chamomile interacts with.

Theoretically, German chamomile might inhibit CYP3A4 and increase levels of drugs metabolized by these enzymes.
In vitro evidence shows that German chamomile might inhibit CYP3A4. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP3A4 in patients taking German chamomile.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your body uses an enzyme called CYP3A4 to help break down estrogen. In lab dish studies, German chamomile appears to slow that same enzyme down a bit. If that happened in real life, estrogen could clear a little more slowly and build up slightly, which in theory might nudge up estrogen-related effects like breast tenderness, bloating, nausea, headaches, or spotting.

The honest part though: this is only seen in test tube experiments, not in actual people, so it may not matter at all, especially with a calming cup of chamomile tea now and then. Estrogen is not a prodrug, so the direction is toward more, not less. If you drink chamomile daily or take concentrated capsules, just mention it to your pharmacist or doctor so they can keep an eye on things.

ESTROGENS

2 of 3

Estrogen also falls under ESTROGENS.

Theoretically, large amounts of German chamomile might reduce the effectiveness of estrogens.
In vitro, German chamomile has demonstrated antiestrogenic activity. Theoretically, large amounts of German chamomile might interfere with hormone replacement therapy through competition for estrogen receptors.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with chamomile and your estrogen therapy. In lab dish studies, compounds in chamomile (like apigenin) seem to gently compete with estrogen at its receptors, the very spots your hormone therapy is meant to act on. In theory, large amounts of chamomile could blunt how well your estrogen works, meaning symptoms like hot flashes, night sweats, or vaginal dryness might creep back.

That said, I want to keep this in perspective. This is theoretical and only seen in test tubes, not in real patients. A cup or two of chamomile tea is very unlikely to cause trouble. The concern is more about concentrated supplement doses taken regularly. If you enjoy chamomile often, just mention it to your pharmacist or doctor so they can keep an eye on things.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

3 of 3

Estrogen also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, German chamomile might inhibit CYP1A2 and increase levels of drugs metabolized by these enzymes.
In vitro and animal research shows that German chamomile might inhibit CYP1A2. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP1A2 in patients taking German chamomile.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: your body uses a liver enzyme called CYP1A2 to help break down estrogen. German chamomile might slow that enzyme down a little. If it does, estrogen could clear out of your body more slowly and build up slightly, which in theory could mean more estrogen-type effects like breast tenderness, bloating, nausea, or headaches.

Now the important part: this is only a theoretical concern from lab and animal studies. It has never actually been reported in people, and chamomile tea is a pretty gentle, everyday drink. An occasional cup is very unlikely to cause trouble. If you sip chamomile daily or take concentrated supplement capsules, just mention it to your pharmacist or doctor so they can keep an eye on things.

German Chamomile
  • Anxiety and stress
  • Sleep and relaxation
  • Digestive upset
  • Skin irritation and inflammation
  • Mouth sores and gum irritation

German chamomile is a widely used herbal remedy taken mainly as a tea for calming, sleep, and digestive complaints. Early research suggests possible benefits for mild anxiety and some skin or digestive issues, but the evidence is limited and most uses are based on tradition rather than strong proof. It is generally well tolerated, but people allergic to ragweed or related plants and those who are pregnant should be cautious.
