Chanca Piedra and Estrogen

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Chanca Piedra interacts with whole categories of medication. Estrogen falls into 2 of those categories, so it can interact with Chanca Piedra in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 2

Why this applies to Estrogen

Estrogen falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: your body uses a liver enzyme called CYP1A2 to help break down estrogen. In lab studies (test tubes, not people), chanca piedra seemed to speed up that enzyme. If that happened in your body, estrogen could get cleared a bit faster, which might mean lower levels and weaker results, things like hot flashes, night sweats, or vaginal dryness creeping back.

I want to be honest about the strength of this, though. This is only theoretical, based on lab data, and it has never actually been reported in real patients. So it's worth a quick mention, not a reason to panic. If you'd like to use chanca piedra, just check with your pharmacist or doctor first so we can keep an eye on how you're feeling.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Estrogen also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your body uses an enzyme called CYP3A4 to help break down estrogen. In lab studies (test tube only), chanca piedra appears to slow that enzyme down. If that happened in real life, estrogen could clear more slowly and build up a bit, which in theory means stronger estrogen effects and possibly more of the usual side effects like breast tenderness, bloating, nausea, headaches, or spotting.

I want to be honest about how soft this evidence is. This has not been seen in actual people, it's purely a theoretical concern from cells in a dish. So I wouldn't panic, but I would loop in your pharmacist or doctor before combining them, just so someone who knows your full picture can weigh in.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
