German Chamomile and Hydrocodone

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

German Chamomile interacts with whole categories of medication. Hydrocodone falls into 3 of those categories, so it can interact with German Chamomile in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CNS DEPRESSANTS

1 of 3

Why this applies to Hydrocodone

Hydrocodone falls under CNS DEPRESSANTS — the group German Chamomile interacts with.

Theoretically, German chamomile might have additive effects when used with CNS depressants.
German chamomile has mild sedative effects. Theoretically, concomitant use with drugs with sedative properties can cause additive effects and side effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: chamomile tea can make you a little drowsy on its own, and hydrocodone is a strong opioid that already causes sleepiness. Put them together and those calming effects can stack up. You might feel more tired, foggy, or dizzy than usual.

I want to be honest about the evidence here. This concern comes mostly from lab and animal research, not from solid human studies, so it's more theoretical than proven. A cup of chamomile tea now and then is unlikely to cause trouble for most people. Still, since hydrocodone can slow your breathing, I'd be cautious with concentrated chamomile supplements and avoid mixing in alcohol or other sedatives. Check with your pharmacist or doctor before combining them.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

2 of 3

Hydrocodone also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, German chamomile might inhibit CYP2D6 and increase levels of drugs metabolized by these enzymes.
In vitro evidence shows that German chamomile might inhibit CYP2D6. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP2D6 in patients taking German chamomile.

Here's a subtle wrinkle with hydrocodone. Part of its pain-relieving punch comes from being converted by an enzyme called CYP2D6 into hydromorphone, a more potent active form. German chamomile has shown, in lab and test-tube studies, that it might slow down that same enzyme. So in theory, chamomile could actually lower how much active painkiller your body makes, meaning you might notice less pain relief rather than more.

The honest truth is this is only a theoretical concern based on lab data, never confirmed in real people, and an occasional cup of chamomile tea is unlikely to cause trouble. If you rely on hydrocodone around the clock and want to use concentrated chamomile supplements, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Dr. Brian Staiger, PharmD, BCPS

Here's the short version: your body uses an enzyme called CYP3A4 to help break down hydrocodone. German chamomile, mostly in concentrated supplement form, has shown in lab dish studies that it might slow this enzyme down. If that happened in real life, more hydrocodone could linger in your system, which could mean stronger effects like extra drowsiness, dizziness, constipation, or in a worst case, slowed breathing.

Now the reassuring part: this has only been seen in test tubes, never confirmed in actual people, so the concern is theoretical. An occasional cup of chamomile tea is very unlikely to cause trouble. Still, since hydrocodone is a strong opioid, it's worth a quick chat with your pharmacist or doctor before using regular chamomile supplements alongside it.

From our pharmacist’s overview on the Hydrocodone page

“Most people taking hydrocodone (Zohydro ER) are treating real pain, and pain wrecks sleep, so sedating supplements look tempting. They are the worst pairing on this page: 78 of the 212 interacting ingredients add to hydrocodone's sedation, which with an opioid can mean dangerously slowed breathing.”

About German Chamomile

German Chamomile
  • Anxiety and stress
  • Sleep and relaxation
  • Digestive upset
  • Skin irritation and inflammation
  • Mouth sores and gum irritation

German chamomile is a widely used herbal remedy taken mainly as a tea for calming, sleep, and digestive complaints. Early research suggests possible benefits for mild anxiety and some skin or digestive issues, but the evidence is limited and most uses are based on tradition rather than strong proof. It is generally well tolerated, but people allergic to ragweed or related plants and those who are pregnant should be cautious.
