Ashwagandha and Hydrocodone

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Ashwagandha interacts with whole categories of medication. Hydrocodone falls into 2 of those categories, so it can interact with Ashwagandha in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CNS DEPRESSANTS

1 of 2

Why this applies to Hydrocodone

Hydrocodone falls under CNS DEPRESSANTS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha might increase the sedative effects of CNS depressants.
Ashwagandha seems to have sedative effects. Theoretically, this may potentiate the effects of barbiturates, other sedatives, and anxiolytics.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: ashwagandha can make you feel drowsy and calm on its own. Hydrocodone is a strong opioid pain medicine that already causes sleepiness, slowed breathing, dizziness, and foggy thinking. Put the two together and those calming effects could stack up, leaving you more sedated than you expect.

I want to be honest about the evidence here. This concern is theoretical, based on how ashwagandha seems to act, not on real cases of people running into trouble. Still, with a medicine as powerful as hydrocodone, I'd rather you play it safe. Watch for extra grogginess or trouble staying alert, and please check with your pharmacist or doctor before adding ashwagandha. Don't change your hydrocodone on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Hydrocodone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for hydrocodone and ashwagandha. Hydrocodone is partly processed by a liver enzyme called CYP3A4. Lab studies (just test-tube and animal work, not people) hint that ashwagandha might speed up that enzyme. If that happened in real life, your body could clear hydrocodone a little faster, which in theory might make your pain medicine feel less effective than usual.

I want to be honest about how weak this signal is: it's theoretical, rated minor, and never confirmed in humans. For most people it's unlikely to be a noticeable problem. Still, since hydrocodone is a strong pain medicine, it's worth a quick chat with your pharmacist or doctor before adding ashwagandha, especially if your pain control suddenly seems off.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

From our pharmacist’s overview on the Hydrocodone page

“Most people taking hydrocodone (Zohydro ER) are treating real pain, and pain wrecks sleep, so sedating supplements look tempting. They are the worst pairing on this page: 78 of the 212 interacting ingredients add to hydrocodone's sedation, which with an opioid can mean dangerously slowed breathing.”
