Fo-ti + Aspirin, Caffeine
Aspirin, Caffeine brand name: Anacin.
Aspirin, Caffeine falls into 3 drug categories that Fo-ti can interact with; the most serious is rated moderate. Theoretically, fo-ti might increase or decrease the levels and clinical effects of drugs metabolized by CYP1A2.

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Fo-ti interacts with whole categories of medication. Aspirin, Caffeine falls into 3 of those categories, so it can interact with Fo-ti in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 3Why this applies to Aspirin, Caffeine
Aspirin, Caffeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Fo-ti interacts with.
In vitro research suggests that fo-ti might inhibit CYP1A2. Additionally, in vitro research suggests that the degree of CYP1A2 inhibition depends on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, in an animal study, an aqueous extract of fo-ti inhibited CYP1A2 while an alcoholic extract of fo-ti induced CYP1A2. Induction or inhibition of CYP1A2 by fo-ti has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
In a product like Anacin, the caffeine part is the piece that matters here. Your liver uses an enzyme called CYP1A2 to clear caffeine out of your system. Lab studies hint that Fo-ti, especially the raw root, might slow that enzyme down. If that happened in your body, caffeine could linger longer and feel stronger, meaning more jitteriness, a racing heart, trouble sleeping, or an upset stomach. The aspirin half isn't really affected by this pathway.
I want to be honest about the evidence though: this is only seen in test tubes and animals, and animal studies actually went both ways. It has never been shown in real people, so I wouldn't lose sleep over it. Still, if you take Fo-ti regularly, mention it to your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 3Aspirin, Caffeine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with Anacin (the aspirin plus caffeine combo) and Fo-ti. The concern on file is that Fo-ti might slow down a liver enzyme called CYP3A4. The caffeine part of this product is partly handled by that pathway, so in theory caffeine could hang around a bit longer and feel stronger. That might mean more jitteriness, a racing heart, trouble sleeping, or an upset stomach.
I want to keep this in perspective, though. This is only lab-dish data, and animal studies didn't even agree. It has never actually been reported in people, so I'd call it more theoretical than proven. Still, since Fo-ti has its own liver-safety questions, it's worth a quick chat with your pharmacist or doctor before combining them.

ANTICOAGULANT/ANTIPLATELET DRUGS
3 of 3Aspirin, Caffeine also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.
Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), dipyridamole (Persantine), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The product you're asking about pairs aspirin with caffeine. Aspirin is the part that matters here. It already thins the blood a little by making platelets less sticky, so it slows clotting on its own.
Here's the concern with Fo-ti: it has been tied to rare reports of liver injury. Your liver makes the clotting proteins your body needs, so if Fo-ti harms it, your blood can become even thinner than aspirin alone would make it. That could mean easier bruising, nosebleeds, or stomach bleeding from the aspirin. This is based on a few anecdotal cases, not strong studies, so it's not common. Still, please check with your pharmacist or doctor before combining them.

From our pharmacist’s overview on the Aspirin, Caffeine page
“Anacin combines aspirin with caffeine, so supplements can hit it from two directions, and a few of the 347 on this list deserve real respect. The biggest group, over 200 ingredients, can raise bleeding risk on top of aspirin's blood-thinning effect; beer and wine carry major ratings because alcohol plus an aspirin-type drug makes stoma…”

About Fo-ti
What is Fo-ti?
- Anti-aging and longevity
- Hair graying and hair loss
- General tonic for vitality
- Constipation
- High cholesterol
Fo-ti (He Shou Wu) is a root used in traditional Chinese medicine, often promoted for healthy aging and hair. High-quality human evidence for these benefits is limited, and processed Fo-ti has been linked to reports of liver injury, so caution and professional guidance are important.
