Why this applies to Codeine Phosphate, Pseudoephedrine

Codeine Phosphate, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti may increase the levels and clinical effects of drugs metabolized by CYP2D6.
Animal research suggests that fo-ti might inhibit CYP2D6. Additionally, an in vitro study suggests that the degree of CYP2D6 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this interaction has not been reported in humans.

Here's the wrinkle with this combo: codeine is actually a prodrug. Your body uses an enzyme called CYP2D6 to convert codeine into morphine, which is what actually relieves pain and quiets a cough. Fo-ti may slow that same enzyme down. So instead of building codeine up, blocking CYP2D6 would likely mean less morphine is made, which can translate to weaker pain or cough relief.

The good news is this is still theoretical. It comes only from lab and animal studies and hasn't been reported in real people, and processed Fo-ti seems to block the enzyme less than the raw root. The pseudoephedrine part isn't really involved here. If you take both and notice your cough medicine isn't working as well, check in with your pharmacist or doctor first.
