Here's the simple version: methyldopa already lowers your blood pressure by relaxing your blood vessels. Gentian, in lab studies, also appears to widen blood vessels a little, which could nudge your pressure down too. Put the two together and the worry is that your blood pressure might drop a touch more than expected.

I want to be honest about how soft this evidence is. It comes from test-tube and animal research, not from real people, so this is theoretical. If anything happened, you'd likely notice the usual signs of pressure dropping low: feeling dizzy, lightheaded, or woozy when you stand up. Occasional culinary bitters before a meal are unlikely to cause trouble, but concentrated daily supplements are worth running past your pharmacist or doctor first.
