Here's the simple version: methyldopa already lowers your blood pressure by relaxing your blood vessels and calming the nerve signals that drive your heart. American hellebore pushes in the very same direction. Its toxic alkaloids drop blood pressure on their own, slow the heart rate, and have a sedating effect. Stack the two together and those effects can pile up.

That means you could see dangerously low blood pressure, a very slow heartbeat, dizziness, fainting, or extreme drowsiness. Honestly, American hellebore is poisonous and isn't a safe supplement to take at all, especially alongside a blood pressure medicine. Please don't combine them, and talk with your pharmacist or doctor before using anything containing this plant.
