CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Fruquintinib
Fruquintinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ginkgo interacts with.
There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with these two. Fruquintinib (Fruzaqla) is a cancer medicine used for certain colorectal tumors, and your liver clears it partly through an enzyme called CYP3A4. Ginkgo might speed that enzyme up in some people. If it does, your body could clear fruquintinib a little faster, which could mean less of the drug is around to do its job. That's not something you want to risk with a cancer treatment.
I'll be honest with you though, the evidence here is shaky and mostly theoretical. Studies on ginkgo and CYP3A4 drugs actually conflict, and some show no real effect. Still, because fruquintinib matters so much, please loop in your oncology team or pharmacist before adding ginkgo. Don't change your fruquintinib on your own.

What Is Fruquintinib Used For?
Brand name: Fruzaqla
Fruzaqla (fruquintinib) is used to treat adults with metastatic colorectal cancer (mCRC) — colorectal cancer that has spread to other parts of the body. It is specifically for patients who have already been treated with several standard chemotherapy regimens and other targeted therapies.
Fruzaqla is intended for patients who have previously received fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and — if their tumor is RAS wild-type and it is medically appropriate — an anti-EGFR therapy.
