Gabapentin enacarbil falls under ANTICONVULSANTS — the group Ginkgo interacts with.

Theoretically, ginkgo might reduce the effectiveness of anticonvulsants.
Ginkgo seeds contain ginkgotoxin. Large amounts of ginkgotoxin can cause neurotoxicity and seizure. Ginkgotoxin is present in much larger amounts in ginkgo seeds than leaves. Ginkgo leaf extract contains trace amounts of ginkgotoxin. The amount of ginkgotoxin in ginkgo leaf and leaf extract seems unlikely to cause toxicity. However, there are anecdotal reports of seizure occurring after use of ginkgo leaf both in patients without a history of seizure disorder and in those with previously well-controlled epilepsy.

Here's the gist with Horizant (gabapentin enacarbil) and ginkgo. The concern isn't really about ginkgo changing how your body processes the drug. Instead, ginkgo seeds contain a natural compound called ginkgotoxin that, in large amounts, can actually lower the seizure threshold. Since Horizant is sometimes used to help control seizures, the worry is that ginkgo could work against it and make a seizure more likely to slip through.

I want to be honest about the strength of this, though. It's largely theoretical and based on a few scattered anecdotal reports. Ginkgo leaf extract (what's in most supplements) has only trace amounts of ginkgotoxin, far less than the seeds. Still, if you take Horizant for seizures, please run ginkgo by your pharmacist or doctor first before adding it.

The evidence for these uses is mixed and generally weak, and it is not proven to prevent memory loss or dementia. About Gabapentin

What Is Gabapentin Used For?

Brand name: Horizant

Gabapentin (brands include Neurontin, Relgaabi, Gabarone, Gralise, and Horizant) is primarily used to manage postherpetic neuralgia (PHN) — the nerve pain that lingers after a shingles outbreak — in adults. Neurontin, Relgaabi, Gabarone, and gabapentin capsules, tablets, and oral solution are also used as add-on therapy for partial onset seizures (a type of epilepsy) in adults and children aged 3 and older.

Horizant (gabapentin enacarbil extended-release tablet) has two specific approvals: treatment of moderate-to-severe primary restless legs syndrome (RLS) in adults, and management of postherpetic neuralgia in adults. Horizant is not recommended for people who need to sleep during the day and stay awake at night. Gralise is approved only for postherpetic neuralgia and cannot be substituted for other gabapentin products due to its different release profile.
