CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

Mavacamten falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Ginkgo interacts with.
Some clinical research shows that a specific ginkgo leaf extract (Remembrance, Herbs Product LTD) 140 mg twice daily can induce CYP2C19 enzymes and potentially decrease levels of drugs metabolized by these enzymes. However, other clinical research shows that taking ginkgo 120 mg twice daily for 12 days has no effect on levels of drugs metabolized by CYP2C19.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist with Camzyos (mavacamten): your body clears it largely through a liver enzyme called CYP2C19. Some research suggests ginkgo may speed that enzyme up, which could clear mavacamten a bit faster and lower its levels in your blood. In plain terms, that might make it work less well at keeping your heart muscle relaxed, which is the whole point of taking it.
That said, the evidence here is shaky and mixed. One ginkgo product seemed to rev up the enzyme, while another study showed no effect at all, so this is more theoretical than proven. Because mavacamten is carefully dosed and monitored with echocardiograms, even small changes matter. Please don't start or stop ginkgo on your own. Run it by your pharmacist or cardiologist first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Mavacamten also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Camzyos (mavacamten). Your body relies on a liver enzyme called CYP3A4 to help clear this medicine. Ginkgo might nudge that enzyme to work a little faster, which could in theory lower your mavacamten levels and make it less effective at controlling your hypertrophic cardiomyopathy symptoms. Mavacamten is not a prodrug, so it needs steady blood levels to do its job.
The honest truth is that the evidence here is shaky. Studies disagree on whether ginkgo speeds up or slows down CYP3A4, and most show little effect. Still, because mavacamten is a heart drug that's carefully dose-adjusted, I'd loop in your cardiologist or pharmacist before adding ginkgo rather than guessing.
