Why this applies to Netupitant, Palonosetron (Oral)
Netupitant, Palonosetron (Oral) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ginkgo interacts with.
There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Akynzeo (netupitant plus palonosetron). The netupitant part is broken down by a liver enzyme called CYP3A4. On paper, ginkgo might speed that enzyme up a little, which could lower netupitant levels and make it slightly less effective at holding back chemo-related nausea and vomiting. That matters because you usually take this medicine right before chemo to keep those symptoms under control.
That said, the evidence here is weak and conflicting. Most studies show ginkgo doesn't meaningfully change CYP3A4, with only a couple of unusual case reports suggesting it can. So this is more of a theoretical concern than a proven problem. Since you're going through chemotherapy, please run any ginkgo use past your oncology team or pharmacist before taking it.

What Is Netupitant and Palonosetron Used For?
Brand name: Akynzeo
Netupitant and palonosetron helps prevent nausea and vomiting from cancer chemotherapy in adults. It is used together with dexamethasone, a steroid medicine. It covers both the first day (acute phase) and the days after (delayed phase).
Akynzeo capsules are used for initial and repeat courses of chemotherapy, including highly emetogenic (strongly nausea-causing) chemotherapy. Akynzeo for injection and Akynzeo injection are used for highly emetogenic chemotherapy only. They have not been studied with anthracycline plus cyclophosphamide chemotherapy.
