Why Are There 2 Interactions?
Ginkgo interacts with whole categories of medication. Paclitaxel falls into 2 of those categories, so it can interact with Ginkgo in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Paclitaxel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ginkgo interacts with.
There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).

Here's the gist if you're getting paclitaxel (Taxol or Abraxane) and thinking about ginkgo. Your liver uses an enzyme called CYP3A4 to help process paclitaxel, and ginkgo might nudge that enzyme. The catch is that the evidence is messy and mostly theoretical. Some studies show ginkgo doesn't change levels of similar drugs at all, while a couple of case reports hinted it could speed up CYP3A4 and lower a drug's levels.
If that did happen with paclitaxel, the worry would be slightly weaker cancer treatment, which is a big deal. Paclitaxel is not a prodrug, so this isn't about boosting side effects. Honestly, during chemo I'd skip ginkgo unless your oncology team clears it, partly because it can also thin the blood. Please run it by them first.

P-GLYCOPROTEIN SUBSTRATES
2 of 2Paclitaxel also falls under P-GLYCOPROTEIN SUBSTRATES.
A small clinical study in healthy volunteers shows that using ginkgo leaf extract 120 mg orally three times daily for 14 days can increase levels of the P-glycoprotein substrate, talinolol, by 36% in healthy male individuals. However, single doses of ginkgo do not have the same effect.

Here's the idea behind this pairing. Paclitaxel is moved around and pushed out of your cells by a pump called P-glycoprotein. Ginkgo may slow that pump down a bit, and if it does, a little more paclitaxel could linger in your body instead of being cleared the usual way. With a strong chemo drug like this, even modest increases can matter, since higher levels can mean more of its typical side effects, like low blood counts, infections, nerve tingling or numbness in the hands and feet, mouth sores, and fatigue.
To be fair, this is mostly theoretical. The human data comes from a different pump-pumped drug, not paclitaxel itself, and ginkgo can also thin the blood. Please don't start or stop anything on your own. Just loop in your oncology team or pharmacist before using ginkgo during treatment.

What Is Paclitaxel (with albumin) Injection Used For?
Brand names: Taxol, Abraxane
Paclitaxel Protein-Bound Particles for Injectable Suspension (Albumin-Bound) is used to treat three types of cancer. It treats metastatic breast cancer in patients whose prior chemotherapy has stopped working or who relapsed within 6 months of completing adjuvant (post-surgery) chemotherapy. It is also used as a first-line treatment for locally advanced or metastatic non-small cell lung cancer (NSCLC) — given together with carboplatin — in patients who cannot have surgery or radiation therapy.
Additionally, it is approved as a first-line treatment for metastatic adenocarcinoma of the pancreas, given in combination with gemcitabine. All three uses apply specifically to Paclitaxel Protein-Bound Particles for Injectable Suspension (Albumin-Bound), including the brands Abraxane and related albumin-bound formulations.
