CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Ripretinib
Ripretinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ginkgo interacts with.
There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your cancer medicine ripretinib (Qinlock) is processed by a liver and gut enzyme called CYP3A4. Ginkgo's possible effect here is on the side of speeding up that enzyme, at least in a few reports. If that happened, your body could clear ripretinib a little faster and leave less of it working against the tumor. With a cancer drug, anything that lowers the level is something we take seriously, because we want it doing its full job.
Honestly though, the evidence is shaky and conflicting. Studies in people often show ginkgo doesn't change CYP3A4 much at all, and this concern is mostly theoretical. Please don't start or stop anything on your own. Just check with your oncology team or pharmacist before adding ginkgo.
