Why this applies to Cisplatin

Cisplatin falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the wrinkle with this one: cisplatin isn't actually a pill. It's given through an IV by your cancer care team, so the worry about glucomannan's thick gel slowing down absorption of a swallowed drug really doesn't apply here. The interaction on file is about oral medicines, where this fiber can blunt how much your body takes in. Cisplatin bypasses your stomach and gut entirely.

That said, I'd still bring it up. People on cisplatin often deal with nausea, vomiting, and kidney strain, and a bulking fiber can affect your gut and how well you stay hydrated. Please check with your oncology team or pharmacist before adding glucomannan during chemo so it fits safely with your treatment.

Glucomannan is a soluble fiber from the konjac root that swells into a gel in your stomach, which may help with mild weight loss, constipation, and modestly lowering cholesterol. It is generally well tolerated when taken with plenty of water, but it must be taken carefully because it can swell and cause choking or blockage if not swallowed properly. About Cisplatin Injection

What Is Cisplatin Injection Used For?

Brand name: Platinol-AQ

Cisplatin Injection is used to treat three types of cancer. For metastatic testicular tumors and metastatic ovarian tumors, it is used as part of combination chemotherapy — alongside other approved cancer medicines — in patients who have already had surgery and/or radiation therapy. For metastatic ovarian tumors, it can also be used alone as a second-line treatment when standard chemotherapy has stopped working and cisplatin has not been tried before.

For advanced bladder cancer (specifically transitional cell bladder cancer) that can no longer be treated with surgery or radiation, Cisplatin Injection is used as a single agent.
