Why this applies to Cisplatin

Cisplatin falls under NEPHROTOXIC DRUGS — the group Rhubarb interacts with.

Theoretically, long-term use of anthraquinones from rhubarb might increase the risk of nephrotoxicity when used with nephrotoxic drugs.
The anthraquinone constituents of rhubarb have been shown to induce nephrotoxicity in animal research. Additionally, in a case report, a 23-year old female presented with kidney failure after taking 6 tablets of a proprietary slimming agent (found to contain the anthraquinones emodin and aloe-emodin from rhubarb) daily for 6 weeks and then adding diclofenac 25 mg 4 times daily for 2 days. The authors postulate that the anthraquinone constituents of rhubarb contributed to the renal dysfunction, and the addition of diclofenac, a nephrotoxic drug, led to renal failure. Until more is known, advise patients to avoid taking rhubarb if they are taking other potentially nephrotoxic drugs.

Here's the concern with this pairing. Cisplatin is already one of the harder chemo drugs on the kidneys, and protecting kidney function is a big part of how it's given (lots of IV fluids, careful monitoring of your blood work). Rhubarb root contains compounds called anthraquinones, and in lab and animal studies those compounds have shown they can stress the kidneys too. Stack two things that both lean on the kidneys, and the worry is added strain.

I'll be honest about the evidence: this is mostly theoretical, based on animal data and one case report involving a different drug, not strong human proof with cisplatin. Still, because cisplatin's kidney risk is so real, I'd play it safe. Please don't start or stop anything on your own. Just run any rhubarb supplement past your oncology team or pharmacist first.

About Cisplatin Injection

What Is Cisplatin Injection Used For?

Brand name: Platinol-AQ

Cisplatin Injection is used to treat three types of cancer. For metastatic testicular tumors and metastatic ovarian tumors, it is used as part of combination chemotherapy — alongside other approved cancer medicines — in patients who have already had surgery and/or radiation therapy. For metastatic ovarian tumors, it can also be used alone as a second-line treatment when standard chemotherapy has stopped working and cisplatin has not been tried before.

For advanced bladder cancer (specifically transitional cell bladder cancer) that can no longer be treated with surgery or radiation, Cisplatin Injection is used as a single agent.
