Selenium and Cisplatin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

IMMUNOSUPPRESSANTS

Why this applies to Cisplatin

Cisplatin falls under IMMUNOSUPPRESSANTS — the group Selenium interacts with.

Theoretically, selenium supplementation may reduce the effectiveness of immunosuppressant therapy.
In vitro research and preliminary clinical evidence suggests that selenium may stimulate the immune system.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the key thing to know: cisplatin isn't really a typical immunosuppressant in the way that label suggests. It's a platinum-based chemotherapy drug that works as an antioxidant's enemy, generating cell-damaging free radicals to kill cancer cells. Because selenium is an antioxidant mineral, the worry on file is that high-dose selenium could, in theory, blunt some of that oxidative damage and make the chemo work a little less effectively.

I want to be honest with you though: this concern comes only from lab and animal studies, so it's theoretical and not well proven in people. Normal dietary amounts of selenium are very unlikely to be an issue. The real risk is taking concentrated selenium supplements during treatment. Please don't start, stop, or change anything on your own. Check with your oncology team or pharmacist first so they can weigh in for your specific plan.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)

From our pharmacist’s overview on the Cisplatin page

“Kidney damage is the risk I would flag first for anyone receiving cisplatin (Platinol-AQ). Cisplatin is a chemotherapy drug that already stresses the kidneys, and two of the three major-rated interactions on this list, Siberian cocklebur and asarabacca, involve herbs that can injure the kidneys on their own, stacking harm on an organ t…”

About Selenium

What is Selenium?

  • Thyroid health support
  • Antioxidant support
  • Immune support
  • General nutrient supplementation
  • Reproductive/fertility support

Selenium is an essential trace mineral your body needs in small amounts for thyroid function, antioxidant defense, and immune health. Most people who eat a varied diet get enough, and supplements are mainly useful for those who are deficient. Because too much selenium can be toxic, stick to recommended amounts and check with your pharmacist or doctor before supplementing.

About Cisplatin Injection

What Is Cisplatin Injection Used For?

Brand name: Platinol-AQ

Cisplatin Injection is used to treat three types of cancer. For metastatic testicular tumors and metastatic ovarian tumors, it is used as part of combination chemotherapy — alongside other approved cancer medicines — in patients who have already had surgery and/or radiation therapy. For metastatic ovarian tumors, it can also be used alone as a second-line treatment when standard chemotherapy has stopped working and cisplatin has not been tried before.

For advanced bladder cancer (specifically transitional cell bladder cancer) that can no longer be treated with surgery or radiation, Cisplatin Injection is used as a single agent.
