Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Greater Celandine interacts with.

Research in vitro shows that chelidonine, a constituent of greater celandine, inhibits cytochrome P450 2D6 (CYP2D6) enzyme activity. Theoretically, greater celandine might increase levels of drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and many others.

Here's the wrinkle with this combo product: the codeine part is what matters most. Codeine is a prodrug, meaning your body has to convert it (using the CYP2D6 enzyme) into morphine before it actually relieves pain. Greater celandine appears to block that very enzyme in lab studies, so in theory less codeine would turn into morphine. That could mean weaker pain relief, leaving you reaching for more.

I want to be honest about how shaky this is: the evidence is only test-tube research, not real patients, so the actual effect in people is unknown. The bigger concern, frankly, is that greater celandine has been tied to serious liver injury. Before pairing it with anything, please check in with your pharmacist or doctor.
