Why this applies to Perphenazine

Perphenazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Greater Celandine interacts with.

Research in vitro shows that chelidonine, a constituent of greater celandine, inhibits cytochrome P450 2D6 (CYP2D6) enzyme activity. Theoretically, greater celandine might increase levels of drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and many others.

Here's the gist for perphenazine: your liver uses an enzyme called CYP2D6 to help clear it out of your body. A compound in greater celandine (chelidonine) appears to block that same enzyme in lab studies. If it does the same thing in people, perphenazine could build up a bit more than usual instead of being broken down at a steady pace.

Since perphenazine is active as-is (not a prodrug that needs switching on), higher levels would mean stronger effects and more side effects, things like drowsiness, dizziness, low blood pressure, and muscle stiffness or restlessness. Keep in mind this is only test-tube evidence, so it's theoretical. I'd also gently mention that greater celandine itself has been tied to serious liver harm. Please check with your pharmacist or doctor before combining them.
