Here's the thing with orlistat and guar gum: both work right inside your gut, not in your bloodstream. Orlistat blocks an enzyme so some of the fat you eat passes through instead of being absorbed. Guar gum is a thick, gel-forming fiber that can grab onto fat and slow how things move through your intestines.

Because they overlap in the same place, the gel could theoretically blunt how well orlistat does its job, or just change how your stomach feels. Honestly, this is a mild, possible concern, not a dangerous one. Both can cause gas, bloating, and loose or oily stools, so taken together you may notice more of that. A simple fix is to separate them, taking guar gum 30 to 60 minutes after your orlistat dose, and check with your pharmacist first.

What Is Orlistat Used For?

Brand names: Xenical, Alli

Xenical and prescription-strength orlistat are approved to help obese adults lose weight and keep it off when used with a reduced-calorie diet. They are intended for people with a body mass index (BMI) of 30 or higher, or 27 or higher if other risk factors such as high blood pressure, diabetes, or dyslipidemia (abnormal cholesterol or fat levels in the blood) are present.

Alli is the over-the-counter version, approved for weight loss in overweight adults 18 and older when used with a reduced-calorie, low-fat diet and exercise program.
