Here's the thing to keep in mind with guar gum and your rifapentine (Priftin). Guar gum is a thick, gel-forming fiber, and once it's in your stomach it can grab onto medications and slow or lower how much of them your body actually absorbs. With a TB drug like rifapentine, you really want full, reliable absorption, because too little drug in your system can mean the medicine doesn't fully knock out the bacteria. That matters a lot when you're treating something like tuberculosis.

This isn't a dangerous reaction, just a timing one. A simple fix is to take your rifapentine first, then wait about 30 to 60 minutes before having guar gum. Food-sized amounts of fiber are far less of a concern than concentrated supplements. Please confirm the spacing with your pharmacist or doctor before changing anything.

Guar gum is a soluble, gel-forming fiber from the guar bean that may modestly help with cholesterol, blood sugar, and bowel regularity. It is generally safe in food amounts, but concentrated supplements can cause gas, bloating, and a risk of throat or gut blockage if not taken with enough water. About Rifapentine

What Is Rifapentine Used For?

Brand name: Priftin

Rifapentine tablets — sold as Priftin and as rifapentine tablets — are used to treat two forms of tuberculosis caused by Mycobacterium tuberculosis. For active pulmonary TB (a live TB infection in the lungs), it is approved for adults and children 12 years and older. It must always be combined with other anti-TB drugs — never taken alone.

For latent tuberculosis infection (LTBI) — where TB bacteria are in the body but not yet causing illness — it is approved for adults and children 2 years and older who are at high risk of the infection becoming active. For LTBI, it is always taken together with isoniazid, once weekly for 12 weeks. Active TB disease must be ruled out before starting LTBI treatment.
