ORAL DRUGS
Why this applies to Sacubitril, Valsartan
Sacubitril, Valsartan falls under ORAL DRUGS — the group Guar Gum interacts with.
Clinical research shows that guar gum reduces or slows absorption of medications such as penicillin and metformin. To avoid changes in absorption, take guar gum 30-60 minutes after oral medications.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: guar gum is a thick, gel-forming fiber, and that gel can grab onto medicine in your stomach and slow or trap part of it before it gets absorbed. With Entresto (sacubitril/valsartan), the concern is that less of your dose might actually make it into your bloodstream, so it may not work as well at protecting your heart and managing your heart failure.
If your heart failure medicine is partly blocked, you could notice your blood pressure isn't as controlled or your symptoms creep back. The good news is timing fixes most of this. A practical move is to take your Entresto first, then wait about 30 to 60 minutes before your guar gum, and always take guar gum with plenty of water. Please run this past your pharmacist or doctor before changing anything.

From our pharmacist’s overview on the Sacubitril, Valsartan page
“Entresto (sacubitril and valsartan) is a heart failure medicine, and heart failure patients tend to be heavy supplement users, reaching for fish oil, potassium, and herbal blood-pressure remedies. That instinct carries real risk here.”

About Guar Gum
What is Guar Gum?
- Constipation and bowel regularity
- Diarrhea (as a stool bulking agent)
- Lowering cholesterol
- Blood sugar control
- Feeling full for weight management
Guar gum is a soluble, gel-forming fiber from the guar bean that may modestly help with cholesterol, blood sugar, and bowel regularity. It is generally safe in food amounts, but concentrated supplements can cause gas, bloating, and a risk of throat or gut blockage if not taken with enough water. Always take it with plenty of fluid and check with your pharmacist or doctor first.
About Valsartan and Sacubitril
What Is Valsartan and Sacubitril Used For?
Brand name: Entresto
Sacubitril and valsartan is used to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure. The benefit is most clearly seen in patients whose heart pumps with a lower-than-normal ejection fraction (the percentage of blood the heart pushes out with each beat).
It is also approved to treat symptomatic heart failure with reduced left ventricular function in children aged 1 year and older. In pediatric patients, it lowers a blood marker of heart stress (NT-proBNP) and is expected to improve heart outcomes.
