Here's the simple version: guar gum is a thick, gel-forming fiber. When you swallow it, it turns into a gel in your stomach that can grab onto pills and slow or reduce how much medicine your body actually absorbs. With Brukinsa (zanubrutinib), the worry is that you'd take in less of your cancer drug than intended, which could make it work less well over time.

Zanubrutinib is not a prodrug, so this is a straightforward absorption issue, not a change in how it's activated. The evidence here comes from other oral drugs like metformin and penicillin, so it's a reasonable concern rather than a proven problem for this exact drug. To stay safe, separate them: take your zanubrutinib first, then wait at least 30 to 60 minutes before the guar gum. Please run the timing past your oncology team or pharmacist before mixing them.

About Zanubrutinib

What Is Zanubrutinib Used For?

Brand name: Brukinsa

Brukinsa (zanubrutinib) is used to treat adult patients with certain blood cancers. It treats mantle cell lymphoma (MCL) in patients who have had at least one prior therapy, Waldenström's macroglobulinemia (WM), relapsed or refractory marginal zone lymphoma (MZL) after at least one prior anti-CD20 treatment, and chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Brukinsa is also used — in combination with another medicine called obinutuzumab — to treat relapsed or refractory follicular lymphoma (FL) in adults who have already received two or more lines of treatment. Several of these uses were granted accelerated FDA approval based on response rates, meaning confirmatory trials are still ongoing.
