Theoretically, guggul might reduce the effects of CYP3A4 substrates.

Here's the gist for ambrisentan (Letairis, Volibris). Your liver clears this medication partly through an enzyme called CYP3A4. Lab and test-tube studies hint that the active compounds in guggul (guggulsterones) may speed that enzyme up. If that holds true in people, your body could clear ambrisentan a bit faster, leaving less of it to do its job.

For a serious condition like pulmonary arterial hypertension, that matters, because weaker drug levels could mean less help with your breathing and exercise tolerance. I want to be honest though: this is only a theoretical concern based on lab data, not real patient studies. Please don't change anything on your own. Just give your pharmacist or doctor a heads-up before adding guggul.

Guggul is a gum resin from the Commiphora wightii tree, long used in Ayurvedic medicine for cholesterol, joint, and skin problems. Modern studies are mixed and often low-quality, and it can cause stomach upset and skin rashes. Talk to your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.

About Ambrisentan

What Is Ambrisentan Used For?

Brand names: Letairis, Volibris

Ambrisentan tablets and Letairis tablets are both used to treat pulmonary arterial hypertension (PAH, WHO Group 1) — a condition where the blood pressure in the arteries leading to the lungs is dangerously high. Both are approved to help adults exercise better and slow the worsening of PAH.

Both ambrisentan tablets and Letairis are also approved to be used together with tadalafil (another PAH medicine) to further reduce the risk of disease progression, lower the chance of hospitalization for worsening PAH, and improve exercise ability. Most of the clinical evidence was in patients with mild-to-moderate symptoms and PAH caused by unknown or inherited causes, or PAH linked to connective tissue diseases.
