Atazanavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.

Theoretically, guggul might reduce the effects of CYP3A4 substrates.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.

Here's the concern with pairing guggul and your HIV combo, Evotaz (atazanavir plus cobicistat). Your body relies on an enzyme called CYP3A4 to handle these drugs. Lab studies suggest the active compounds in guggul (guggulsterones) may speed that enzyme up. If that happens in people, your body could clear atazanavir faster, leaving less of it in your blood.

Why that matters: with an HIV medication, lower drug levels can mean the virus isn't kept in check, and over time that risks treatment failure or resistance. Right now this is only based on test-tube research, not human studies, so we can't say for sure it happens. Still, because this drug keeps your HIV controlled, please don't add guggul without checking with your doctor or pharmacist first.

What Is Atazanavir and Cobicistat Used For?

Brand name: Evotaz

Evotaz (atazanavir/cobicistat) is used to treat HIV-1 infection. It must always be taken together with other antiretroviral medicines — it is not a complete regimen on its own. It is approved for adults and for pediatric patients who weigh at least 35 kg.

In patients who have been on HIV treatment before (treatment-experienced), how well Evotaz works depends on how much resistance to protease inhibitors the virus has already developed. Your prescriber will review this before starting you on Evotaz.
