Irinotecan Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.

Theoretically, guggul might reduce the effects of CYP3A4 substrates.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.

Here's the wrinkle with irinotecan (Onivyde): your liver activates it into its powerful working form, called SN-38, partly through the same CYP3A4 pathway that lab studies suggest guggul may speed up. Because irinotecan is a chemotherapy drug given carefully by your cancer team, even small shifts in how your body handles it can matter. In theory, if guggul revs up that enzyme, it could change your drug levels and either weaken the cancer-fighting effect or alter side effects like diarrhea, low blood counts, nausea, or fatigue.

I want to be honest: this concern comes only from test-tube and animal research, not from people, so it's far from proven. Still, while you're on chemo, please don't add guggul without first clearing it with your oncologist or pharmacist.

  • Irinotecan lipid complex is used in combination with other medications to treat pancreatic cancer that has spread to other parts of the body that has worsened after treatment with other chemotherapy medications. Irinotecan lipid complex is in a class of antineoplastic medications called topoisomerase I inhibitors. It works by stopping the growth of cancer cells.
