Here's the gist with this combo pill (acetaminophen, butalbital, and codeine). Hu Zhang is rich in resveratrol, and in lab dish studies resveratrol can slow down a liver enzyme called CYP1A2. The theory is that this could let certain medicines linger a bit longer in your body. It's worth noting this has never actually been seen happen in real people, so I wouldn't lose sleep over it.

For this specific product, CYP1A2 isn't the main pathway that handles the codeine or butalbital, so the practical effect is likely small. Still, if the herb did slow clearance, you might feel a touch more drowsiness, dizziness, or constipation. Please run it by your pharmacist or doctor before pairing them, especially since this medicine can already make you sleepy.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP2E1.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP2E1 enzyme. Also, a pharmacokinetic study shows that taking resveratrol 500 mg daily for 10 days prior to taking a single dose of chlorzoxazone 250 mg increases the maximum concentration of chlorzoxazone by about 54%, the area under the curve of chlorzoxazone by about 72%, and the half-life of chlorzoxazone by about 35%. Chlorzoxazone is used as a probe drug for CYP2E1.

Here's the piece that matters with this combo pill: the acetaminophen part is partly handled by a liver enzyme called CYP2E1. The resveratrol in Hu Zhang may gently block that enzyme. In theory, that could shift how acetaminophen is processed. The reassuring part is that CYP2E1 is actually the pathway that creates acetaminophen's harmful liver byproduct, so slowing it isn't clearly dangerous, and this whole concern is theoretical, based mostly on lab work and one study using a different test drug.

The codeine and butalbital pieces aren't really driven by CYP2E1, so I wouldn't expect big changes there. Food amounts of knotweed are unlikely to matter; concentrated supplements are the bigger question. Since this is a controlled, narcotic-containing medicine, please run it by your pharmacist or doctor before combining.
