Hu Zhang and Atovaquone, Proguanil

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

Why this applies to Atovaquone, Proguanil

Atovaquone, Proguanil falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP2C19.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP2C19 enzyme. This interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know with Malarone (atovaquone and proguanil) and Hu Zhang. The resveratrol in Hu Zhang can, at least in lab studies, slow down an enzyme called CYP2C19. The proguanil half of Malarone relies partly on that enzyme. Proguanil is actually a prodrug, meaning your body converts it into its active form (cycloguanil) using CYP2C19. So if that enzyme is blocked, you might make less of the active form, which could in theory make the malaria protection a bit weaker.

That said, this is purely theoretical, based on test-tube research, and has never been reported in real people. Because malaria is serious and you don't want a weaker shield, I'd just run it by your doctor or pharmacist before pairing them.

  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

About Atovaquone and Proguanil

What Is Atovaquone and Proguanil Used For?

Brand name: Malarone

Atovaquone and proguanil is used for two purposes: preventing Plasmodium falciparum (P. falciparum) malaria before and during travel to areas where it is present, and treating acute, uncomplicated P. falciparum malaria once infection has occurred. This includes areas where other antimalarials such as chloroquine, mefloquine, halofantrine, or amodiaquine may have unacceptable failure rates due to drug resistance.

These uses apply across the supported products: Atovaquone and Proguanil Hydrochloride tablets, Atovaquone and Proguanil Hydrochloride Pediatric tablets, and Malarone tablets. Pediatric formulations are dosed by body weight for children; adult-strength formulations are used for adults and heavier children.
