Why this applies to Eravacycline

Eravacycline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Here's the thinking on combining Hu Zhang with eravacycline (Xerava). This herb contains resveratrol, and in test-tube studies resveratrol may slow down a liver enzyme called CYP3A4. Since eravacycline is partly broken down by that same enzyme, the theory is that the herb could let a little more of the drug build up in your system. Eravacycline is a fully active antibiotic (not a prodrug), so higher levels would mean a stronger effect and possibly more stomach upset, nausea, or infusion-site reactions.

In real life, this is mostly theoretical. One human study using a big resveratrol dose did not require any medication changes. Also, eravacycline is given in the hospital as an IV for a short course, so the team is already monitoring you. Still, mention any supplements to your pharmacist or doctor.

What Is Eravacycline Injection Used For?

Brand name: Xerava

Xerava (eravacycline) is used to treat complicated intra-abdominal infections (serious infections inside the abdomen, such as those following bowel problems or surgery) in adults 18 and older. It targets a range of bacterial types, including E. coli, Klebsiella pneumoniae, Enterococcus species, Staphylococcus aureus, Bacteroides species, and several others identified by lab testing.

Xerava is not approved for complicated urinary tract infections (cUTI). It should only be used when a bacterial infection is proven or strongly suspected — using antibiotics unnecessarily contributes to antibiotic resistance.
