Hu Zhang and Ethinyl estradiol, Levonogestrel

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Hu Zhang interacts with whole categories of medication. Ethinyl estradiol, Levonogestrel falls into 3 of those categories, so it can interact with Hu Zhang in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Ethinyl estradiol, Levonogestrel

Ethinyl estradiol, Levonogestrel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP1A2.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP1A2 enzyme. This interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist for your birth control pill, Lybrel (ethinyl estradiol and levonorgestrel). One compound in Hu Zhang, resveratrol, has been shown in test-tube studies to slow down a liver enzyme called CYP1A2, which helps process the estrogen part of your pill. In theory, if that enzyme is blocked, more estrogen could hang around in your body. That might mean a bit more estrogen-type side effects, like nausea, breast tenderness, headaches, or bloating.

But please keep this in perspective. This is only a lab-based idea, and it has never actually been seen in real people. The effect, if any, is likely small. If you'd like to use a concentrated Hu Zhang supplement, just run it by your pharmacist or doctor first so they can keep an eye on things.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 3

Ethinyl estradiol, Levonogestrel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your birth control pill, Lybrel, contains two hormones (ethinyl estradiol and levonorgestrel) that your liver clears using an enzyme called CYP3A4. The thinking here is that resveratrol in Hu Zhang might slow that enzyme down a little. In theory, that could let a bit more of the hormones linger in your system, which might mean slightly more of the usual estrogen-type side effects like nausea, breast tenderness, headaches, or bloating.

The good news: this is only a theoretical concern based on lab studies. When researchers gave people a hefty 3000 mg of resveratrol daily for eight weeks, they did not need to adjust any CYP3A4 medications. So a real-world problem seems unlikely. Still, since this affects your contraception, it is worth a quick check with your pharmacist or doctor before combining them.

ESTROGENS

3 of 3

Ethinyl estradiol, Levonogestrel also falls under ESTROGENS.

Theoretically, hu zhang might competitively inhibit the effects of estrogen replacement therapy.
In vitro research shows that hu zhang might have estrogenic activity.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the short version about taking Hu Zhang alongside your Lybrel (ethinyl estradiol with levonorgestrel). This herb is loaded with resveratrol and related plant compounds that, in test-tube studies, seem to act a bit like estrogen and may compete with the estrogen in your pill. The worry is that, in theory, it could blunt how well your birth control works.

I want to be honest about how thin the evidence is. This comes from lab and animal research only, not from real people, so it's more of a question mark than a proven problem. Still, since reliable contraception matters, I wouldn't want to risk anything that could lead to breakthrough bleeding or a missed pill effect. Please run it by your pharmacist or doctor before combining them.

About Hu Zhang

What is Hu Zhang?

  • Source of resveratrol
  • Heart and circulation support
  • Inflammation
  • Liver and digestive complaints (traditional)
  • Cough and respiratory symptoms (traditional)
  • Skin and joint conditions (traditional topical use)

Hu Zhang (Japanese knotweed root) is a traditional Chinese herb that is one of the richest natural sources of resveratrol and emodin. Some lab and early human research looks interesting for inflammation and heart health, but high-quality evidence in people is limited. Talk to your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.
