Hu Zhang and Nelfinavir Mesylate

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Hu Zhang interacts with whole categories of medication. Nelfinavir Mesylate falls into 2 of those categories, so it can interact with Hu Zhang in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

1 of 2

Why this applies to Nelfinavir Mesylate

Nelfinavir Mesylate falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP2C19.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP2C19 enzyme. This interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Hu Zhang and your HIV medicine nelfinavir (Viracept). Hu Zhang is loaded with resveratrol, and in test-tube studies resveratrol can slow down a liver enzyme called CYP2C19, one of the pathways that helps clear nelfinavir. If that enzyme gets blocked, nelfinavir levels could theoretically creep up, which might mean more of its usual side effects like diarrhea, nausea, gas, or stomach upset.

The honest part: this is only a theoretical concern based on lab work, and it has not actually been seen in real people. Still, since staying steady on HIV therapy matters so much, please don't add Hu Zhang without first checking with your pharmacist or doctor. They can tell you if it's a good fit for you.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Nelfinavir Mesylate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea: your body uses an enzyme called CYP3A4 to break down nelfinavir (Viracept). Hu Zhang contains resveratrol, which in lab dishes can slow that enzyme down. If that happened in real life, nelfinavir could build up a little more than expected, which might mean more of its usual side effects like diarrhea, nausea, gas, or stomach upset.

I want to be honest about how shaky this is, though. This concern comes from test-tube data only, and a human study using a hefty 3,000 mg of resveratrol daily didn't force any dose changes to CYP3A4 medications. Because keeping HIV medicine working correctly really matters, don't add Hu Zhang without first checking with your pharmacist or HIV doctor.

What is Hu Zhang?

  • Source of resveratrol
  • Heart and circulation support
  • Inflammation
  • Liver and digestive complaints (traditional)
  • Cough and respiratory symptoms (traditional)
  • Skin and joint conditions (traditional topical use)

Hu Zhang (Japanese knotweed root) is a traditional Chinese herb that is one of the richest natural sources of resveratrol and emodin. Some lab and early human research looks interesting for inflammation and heart health, but high-quality evidence in people is limited. Talk to your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.
