Tramadol, Acetaminophen falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hu Zhang interacts with.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP1A2 enzyme. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Tramacet (tramadol plus acetaminophen) and Hu Zhang. This herb is rich in resveratrol, and in test-tube studies resveratrol seems to slow down a liver enzyme called CYP1A2. If that enzyme works more slowly, drugs it helps clear could hang around a bit longer in your body. In theory, that might nudge up the effects of tramadol, things like drowsiness, dizziness, nausea, or constipation.
That said, I want to be honest about how soft this is. This is a theoretical concern from lab work only, and it hasn't actually been seen in real people. CYP1A2 isn't even tramadol's main pathway. So I wouldn't panic, but since both can cause sedation, it's worth a quick chat with your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Here's the short version: Tramadol leans on liver enzymes (including CYP3A4) to work properly. The resveratrol in Hu Zhang might, at least in a test tube, slow down CYP3A4 a bit. If that happened in your body, more tramadol could hang around, which could nudge up its usual effects like drowsiness, dizziness, nausea, or constipation. The acetaminophen half isn't really the concern here.
I want to keep this in perspective, though. This is a theoretical worry based on lab work, and a human study using a big resveratrol dose didn't force any dose changes for CYP3A4 drugs. Still, since tramadol affects how alert you feel, it's worth running it by your pharmacist or doctor before combining the two.

Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP2E1 enzyme. Also, a pharmacokinetic study shows that taking resveratrol 500 mg daily for 10 days prior to taking a single dose of chlorzoxazone 250 mg increases the maximum concentration of chlorzoxazone by about 54%, the area under the curve of chlorzoxazone by about 72%, and the half-life of chlorzoxazone by about 35%. Chlorzoxazone is used as a probe drug for CYP2E1.

Here's the thing about this Tramacet combo and Hu Zhang. The resveratrol in Hu Zhang may gently block an enzyme called CYP2E1. That enzyme matters mostly for the acetaminophen side of this pill. Normally a small fraction of acetaminophen gets turned by CYP2E1 into a harsh byproduct that's tough on the liver. By slowing that pathway a bit, Hu Zhang could actually shift acetaminophen toward safer routes, so this isn't a dramatic danger.
Honestly, this is theoretical and based on lab work and one study using a different probe drug, not on Tramacet itself. Tramadol is cleared mainly by other enzymes, so it's largely unaffected here. Just keep your acetaminophen total under the daily limit and check with your pharmacist before adding Hu Zhang.
