Dextromethorphan HBr, Guaifenesin falls under SEROTONERGIC DRUGS — the group Iboga interacts with.

Theoretically, taking iboga with serotonergic drugs might increase levels of serotonin, resulting in additive effects with serotonergic drugs.
Ibogaine, a constituent in iboga, has serotonergic effects. Theoretically, combining serotonergic drugs with iboga might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using iboga with serotonergic drugs.

Here's the connection worth knowing about. The dextromethorphan in your cough medicine nudges serotonin activity in the brain, and Iboga (through its ibogaine content) does the same thing. Stacking two serotonin-boosting things together can, in theory, push serotonin too high. That's called serotonin syndrome, and warning signs include agitation, a racing heart, shivering, sweating, muscle twitching, or confusion. The guaifenesin part just loosens mucus and isn't really involved here.

Honestly, this concern is theoretical and based on how these substances work, not on real cases with this combo. But Iboga carries its own serious heart rhythm dangers and is illegal in the U.S., so I wouldn't use it at all. If you ever feel those symptoms, please check in with your pharmacist or doctor right away.
