Givosiran Sodium falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS — the group Iboga interacts with.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing to understand: in this pairing, it's the Iboga that gets affected, not your Givlaari. Givosiran (Givlaari) is a once-monthly injection used for acute hepatic porphyria, and one way it works is by lowering the activity of certain liver enzymes, including CYP2D6. Ibogaine, the active part of Iboga, is broken down by that same CYP2D6 enzyme.

So if CYP2D6 is slowed down, ibogaine can build up in the body and hit harder. That matters because Iboga already carries serious risks, including dangerous heart rhythm changes. This concern is mostly theoretical, based on a similar drug, but I'd take it seriously. On top of that, Iboga is illegal in the U.S. and not something I'd ever recommend. Please talk with your doctor or pharmacist before mixing anything with Givlaari.
