Niraparib, Abiraterone falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS — the group Iboga interacts with.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing with this pairing: it's the Iboga that gets affected, not your Akeega. The abiraterone part of Akeega is a known blocker of the CYP2D6 enzyme, and Iboga's active compound (ibogaine) is cleared by that same enzyme. So if you took them together, your body would break down ibogaine more slowly and its levels could climb, possibly doubling based on a small human study using a different CYP2D6 blocker.

Why that matters: ibogaine can trigger dangerous heart rhythm changes, and abiraterone can also affect the heart and potassium. Stacking them could raise that risk. This is largely theoretical, but Iboga is illegal in the US and genuinely unsafe. Please don't combine them, and talk with your oncology team or pharmacist about any supplement.
