Why this applies to Pyridostigmine Bromide
Pyridostigmine Bromide falls under CHOLINERGIC DRUGS — the group Iboga interacts with.
In vitro research shows that iboga can inhibit acetylcholinesterase and might increase acetylcholine levels. However, the constituent ibogaine does not appear to have this effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with these two together. Pyridostigmine works by blocking an enzyme called acetylcholinesterase, which lets more of the nerve signal acetylcholine build up so your muscles work better. Some lab (test tube) studies suggest Iboga might block that same enzyme. If both are pushing in the same direction, you could in theory get too much cholinergic activity.
What that might look like in real life: more drooling, watery eyes, stomach cramps, diarrhea, sweating, a slow heartbeat, or muscle twitching. I want to be honest though, this is theoretical, based only on lab work, and the main Iboga compound (ibogaine) didn't show this effect. The bigger concern is that Iboga itself is illegal and risky for the heart. Please talk with your pharmacist or doctor before mixing anything with your Mestinon.

Pyridostigmine is used for different purposes depending on the product.
Mestinon and pyridostigmine bromide oral solution are used to treat myasthenia gravis. Regonol injection reverses the effects of nondepolarizing muscle relaxants. Pyridostigmine bromide tablets and extended-release tablets are for military medical use only, as pretreatment against the lethal effects of soman nerve agent poisoning in adults.
