Here's the gist for Gilotrif (afatinib): your body relies on a kind of cellular pump called P-glycoprotein to help move this drug along and limit how much builds up inside you. The piperine in Indian long pepper may slow that pump down a bit. In theory, that could let more afatinib hang around, which might nudge up its more common side effects, things like diarrhea, mouth sores, an acne-like rash, and dry or cracked skin.

I want to be honest about the strength of this, though. It comes from lab (test-tube) studies, not real patients, so it's more of a caution than a proven problem. Casual culinary use of pepper is unlikely to matter much, but regular concentrated supplements are worth mentioning. Please loop in your oncology pharmacist or doctor before adding it.

What Is Afatinib Used For?

Brand name: Gilotrif

Gilotrif (afatinib) is used to treat metastatic non-small cell lung cancer (NSCLC) — cancer that has spread and cannot be removed by surgery. It has two specific approved uses.

First, Gilotrif is used as an initial treatment for metastatic NSCLC when the tumor carries certain EGFR mutations (gene changes in the cancer cells) confirmed by an FDA-approved test — specifically non-resistant mutations. Second, Gilotrif is used to treat metastatic squamous NSCLC (a particular cell type of lung cancer) in patients whose cancer progressed during or after platinum-based chemotherapy.
