Indian Long Pepper and Amoxicillin, Clavulanate Potassium

Here is the interaction we found between these two, with the evidence behind it.

Minor

AMOXICILLIN (Amoxil, Trimox)

Theoretically, Indian long pepper might increase the effects and adverse effects of amoxicillin.
Evidence from animal research shows that piperine, a constituent of Indian long pepper, increases the plasma levels of amoxicillin when taken concomitantly.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Augmentin (which pairs amoxicillin with clavulanate). The piperine in Indian long pepper can slow how fast your body clears certain medicines, and in animal studies it nudged amoxicillin blood levels a bit higher. In theory, that could mean slightly stronger amoxicillin effects, which usually shows up as more of the everyday side effects you already might get from Augmentin, like an upset stomach, loose stools, or nausea.

That said, this is a minor, theory-based interaction seen mostly in animal research, not something I'd lose sleep over. A sprinkle of pepper on food is very unlikely to matter. If you take concentrated pippali supplements regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.

From our pharmacist’s overview on the Amoxicillin, Clavulanate Potassium page

“Beer and wine sit among the six major-rated interactions for amoxicillin and clavulanate potassium (Augmentin), and this list deserves real caution. Clavulanate is a well-known antibiotic cause of liver injury, so the 38 ingredients flagged for adding liver stress carry extra weight with this particular drug.”
