Estradiol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with estradiol and Indian long pepper. Your body uses an enzyme called CYP3A4 to help break estradiol down. Lab studies suggest piperine, the active compound in long pepper, can slow that enzyme a bit. In theory, that could let more estradiol linger in your system, which might mean stronger effects or more side effects like breast tenderness, bloating, nausea, headaches, or spotting.
I want to be honest though: this is based only on test-tube research, not real people, so it may not amount to much in practice. A sprinkle of pepper on your food is very unlikely to matter. If you take concentrated long pepper supplements regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.

P-GLYCOPROTEIN SUBSTRATES

Here's the gist with Indian long pepper and estradiol. The piperine in this spice may block a transporter called P-glycoprotein, which helps move certain drugs in and out of your body. In a test-tube setting, blocking that pump could let a bit more estradiol stay in your system, nudging blood levels up.
If your estradiol levels rose, you might notice more of its usual effects, things like breast tenderness, bloating, nausea, or spotting. That said, this is purely theoretical, based on lab data, not real patients. A sprinkle of pepper on food is very unlikely to matter. Concentrated pippali supplements are the bigger question mark. If you take it regularly, just give your pharmacist or doctor a heads-up so they can keep an eye on things.
