In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Your body uses an enzyme called CYP3A4 to help break down conjugated estrogens. Piperine, the active compound in Indian long pepper, may slow that enzyme down. If it does, more estrogen could linger in your blood, which in theory could bump up both the benefits and the side effects. With estrogen, that might mean things like breast tenderness, bloating, nausea, headaches, or spotting.
I want to be honest about the strength of this, though. It's only a theoretical concern based on lab studies, not real people, and a sprinkle of pepper in your food almost certainly won't matter. Concentrated pippali supplements taken daily are a different story. If you use one regularly, just give your pharmacist or doctor a heads-up so they can keep an eye on things.

Here's the basic idea with Premarin and Indian long pepper: your body uses a kind of cellular bouncer called P-glycoprotein to help move certain medications out of your system. Lab studies suggest that piperine, the spicy compound in long pepper, can slow that bouncer down. Since conjugated estrogens are handled in part by this pathway, the theory is that more estrogen could stick around in your blood than usual.
I want to be honest about how soft this evidence is. This comes from test-tube research, not real people, so we don't actually know if it matters day to day. If estrogen levels did creep up, you might notice things like breast tenderness, bloating, nausea, or spotting. A pinch of the spice in food likely won't move the needle, but if you take concentrated long pepper supplements regularly, mention it to your pharmacist or doctor.
