Indian Long Pepper and Triazolam

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Triazolam

Triazolam falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.

Theoretically, Indian long pepper might increase the effects and adverse effects of CYP3A4 substrates.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: your body relies on a liver enzyme called CYP3A4 to break down triazolam (Halcion) and clear it out. The piperine in Indian long pepper may slow that same enzyme down. If the drug clears more slowly, it can build up in your blood, which could make triazolam's effects stronger than expected, think extra drowsiness, grogginess the next morning, dizziness, or feeling unsteady on your feet.

I want to be honest about the evidence, though. This is based on lab (test tube) research, not real patient studies, so it's more of a theoretical caution than a proven problem. A pinch of pepper in cooking is unlikely to matter much, but a concentrated daily supplement is worth mentioning to your pharmacist or doctor before you combine them.
