ANTICOAGULANT/ANTIPLATELET DRUGS
Why this applies to anistreplase, APSAC
anistreplase, APSAC falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Indian Long Pepper interacts with.
In vitro research shows that Indian long pepper extract inhibits platelet aggregation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Anistreplase (Eminase) is a powerful clot-busting medicine, a thrombolytic given in the hospital during a heart attack to dissolve a blocking clot fast. Because its whole job is to break down clots, anything that also thins the blood can stack on top of it. The concern here is added bleeding, which with this drug can mean bruising, oozing at IV sites, or in rare cases more serious internal bleeding.
Now, here's the reassuring part. This caution comes only from lab (test tube) research showing Indian long pepper's piperine may make platelets a little less sticky. That's weak, theoretical evidence, and the spice amounts used in cooking are unlikely to matter. Anistreplase is also given by professionals who monitor you closely. Still, let your doctor or pharmacist know if you take this supplement regularly.

From our pharmacist’s overview on the anistreplase, APSAC page
“Anistreplase (Eminase) is a clot-dissolving drug given during a heart attack, and because bleeding is already its biggest hazard, this list deserves real vigilance. Almost every ingredient here has some blood-thinning or antiplatelet activity, meaning it could stack with anistreplase and make bleeding more likely or harder to stop.”
