Theoretically, Indian long pepper might increase the effects and adverse effects of CYP3A4 substrates.

Here's the gist with Eliglustat (Cerdelga) and Indian long pepper. Your body relies on an enzyme called CYP3A4 to help clear eliglustat from your blood. The piperine in long pepper may slow that enzyme down a bit, so in theory the medicine could linger longer and build up to higher levels than expected.

Why that matters: eliglustat is one of those drugs where higher blood levels can affect your heart's rhythm (the QT interval), so it's not something to take lightly. The good news is this is a theoretical concern, based on lab studies rather than real patients. A sprinkle of pepper in food is very unlikely to do much, but concentrated pippali supplements are a different story. Please check with your pharmacist or doctor before using them regularly.

About Eliglustat

What Is Eliglustat Used For?

Brand name: Cerdelga

Cerdelga (eliglustat) is used for the long-term treatment of Gaucher disease type 1 in adults. Gaucher disease type 1 is a genetic condition in which a fatty substance called glucosylceramide builds up in organs like the liver, spleen, and bone marrow, causing them to enlarge and leading to problems like anemia and low platelet counts.

Cerdelga is only appropriate for adults whose bodies process the drug in a predictable way — specifically those identified as CYP2D6 extensive metabolizers (EMs), intermediate metabolizers (IMs), or poor metabolizers (PMs) by an FDA-cleared genetic test. People who are CYP2D6 ultra-rapid metabolizers may not reach drug levels high enough to benefit, and Cerdelga is not recommended for them.
