Fruquintinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for fruquintinib (Fruzaqla), the pill used to treat certain advanced colorectal cancers. Your liver clears this drug partly through an enzyme called CYP3A4. The piperine in Indian long pepper can slow that enzyme down in the test tube, so in theory fruquintinib could clear more slowly and build up a bit in your blood. Higher levels can mean stronger side effects, think high blood pressure, mouth sores, hand-foot skin reactions, fatigue, or bleeding.
Keep this in perspective, though. This warning is theoretical and comes from lab research, not from people. A pinch used in cooking is unlikely to matter much, but a concentrated daily supplement is a different story. Since fruquintinib is a serious cancer medicine, please loop in your oncology team or pharmacist before adding it.

Fruzaqla (fruquintinib) is used to treat adults with metastatic colorectal cancer (mCRC) — colorectal cancer that has spread to other parts of the body. It is specifically for patients who have already been treated with several standard chemotherapy regimens and other targeted therapies.
Fruzaqla is intended for patients who have previously received fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and — if their tumor is RAS wild-type and it is medically appropriate — an anti-EGFR therapy.
