Nirmatrelvir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.

Theoretically, Indian long pepper might increase the effects and adverse effects of CYP3A4 substrates.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.

Here's the interesting twist with Paxlovid. It's actually two drugs in one, and the ritonavir piece is already a strong CYP3A4 blocker on purpose. Its whole job is to slow down how fast your body clears the nirmatrelvir so the antiviral stays at a higher level. Piperine, the active compound in Indian long pepper, may also gently block this same CYP3A4 pathway, at least based on lab (test-tube) studies.

So in theory, adding the pepper on top of ritonavir could nudge drug levels a bit higher and bump up side effects like an altered taste, nausea, diarrhea, or muscle aches. That said, this is theoretical only, and culinary amounts in food are unlikely to matter. Since Paxlovid is just a short 5-day course, it's worth a quick check with your pharmacist before mixing in concentrated pippali supplements.
