Vorapaxar falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea. Your body relies on a liver enzyme called CYP3A4 to clear vorapaxar (Zontivity). The piperine in Indian long pepper can slow that enzyme down in lab studies, so in theory it could let vorapaxar build up a bit higher than usual. Since vorapaxar is a blood thinner that keeps platelets from clumping, more of it in your system could mean a higher chance of bleeding problems, things like easy bruising, nosebleeds, or bleeding that's slow to stop.
I want to be honest that this is only a theoretical concern based on test-tube work, not real patient cases, and a sprinkle of pepper in cooking is unlikely to matter. Concentrated pippali supplements are the bigger question. Since vorapaxar already affects bleeding, please check with your pharmacist or doctor before using it regularly.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Vorapaxar (Zontivity) is already a strong blood thinner that keeps your platelets from clumping into clots. In lab dish studies, Indian long pepper has shown a mild ability to do the same thing, meaning it may keep platelets from sticking together. Stack that effect on top of vorapaxar, and you could, in theory, tip the balance toward easier bleeding.
What might that look like? Easy bruising, nosebleeds, bleeding gums, or cuts that take longer to stop. I'll be honest, this concern comes only from test-tube research, not from people, so it's far from proven. Still, since you're usually on vorapaxar alongside aspirin and Plavix, your bleeding risk is already elevated. Please check with your pharmacist or doctor before using long pepper supplements regularly. A little in cooking is much less of a worry than concentrated capsules.

Zontivity (vorapaxar) is used to lower the risk of heart attack, stroke, cardiovascular death, and the need for urgent procedures to open blocked arteries. It is prescribed for people who have already had a heart attack or who have peripheral arterial disease (PAD) — a condition where arteries outside the heart become narrowed or blocked.
Zontivity is not used alone. It is given alongside aspirin and/or clopidogrel as part of a broader antiplatelet treatment plan.
